The Church of the Nativity of the Blessed Virgin Mary in Chocianowice is a wooden temple with a log construction erected in the second half of the 17th century. The building stands on the site of a former church that burned down during the Hussite Wars in the 15th century. In the vicinity, there is a new, 20th-century temple.
Above the main part of the church, there is a tower covered with an octagonal shingled roof, while above the nave there is a small turret with a pointed finial. From the northern side, the church is surrounded by "soboty" (traditional wooden arcades).
In the Baroque main altar from the first half of the 18th century, there is a copy of the 17th-century painting of Our Lady of Częstochowa. In the past, the main altar housed an 18th-century painting of Our Lady of Chocianowice, which is currently located in the new church. Above the painting, there is an image of the Holy Trinity, and on the sides – two sculptures of angels, which once constituted an ornament of the 19th-century pipe organs that are currently located in the Diocesan Museum in Opole.
To the left of the entrance to the temple, there is a cross made by an unknown folk sculptor, and next to it – the so-called Spiritual Alms Box (Karbona Duchowa) from the 19th century. The church pipe organs date back to the late 18th century, and the baptismal font in the shape of a cornucopia supported by an angel comes from the first half of the 18th century.
By the northern wall of the old church, there is a monument dedicated to the fallen residents of the village during World War I, while between the old and new temples stands a black marble monument commemorating the locals who died during World War II.
